Pdf Summary
The Cochrane review "Surgical versus non-surgical interventions for treating patellar dislocation" analyzed the effects of surgical and non-surgical treatments for individuals with a first-time patellar dislocation. The review included six studies with a total of 344 participants, all of whom experienced primary patellar dislocation. The quality of the evidence was assessed as very low due to methodological issues and small study sizes.

Key findings from the review indicate that surgical treatment may lead to a reduced risk of recurrent dislocation within two to five years when compared to non-surgical management. However, this potential benefit decreases over longer follow-up periods (six to nine years), where the difference was less clear and non-surgical treatment might still be beneficial for some. The evidence for patient-reported outcomes like knee function scores showed mixed results, with some measures favoring surgery in the short term but not for longer follow-up periods.

Importantly, only one study reported adverse effects, indicating more surgical complications, but the overall frequency of subsequent surgeries was slightly higher in the surgical group. Due to the lack of high-quality evidence and concerns about study design and biases, conclusions are tentative.

The review highlights a need for larger, well-conducted randomized controlled trials to provide more conclusive evidence regarding the best treatment strategies for patellar dislocation. Additionally, future research should distinguish between primary and recurrent dislocations and consider anatomical factors and their impact on treatment outcomes. The current evidence suggests a cautious approach to choosing surgical interventions for primary patellar dislocation unless specific surgical indications are present.
