Pdf Summary
Spondylolysis is a stress fracture of the lower back bone, often affecting the fifth lumbar vertebra (L5), typically on one side. This condition is a common source of back pain in athletes who frequently extend their lower back, such as gymnasts, divers, wrestlers, rowers, and football linemen. The main causes include repetitive hyperextension of the lower back, insufficient rest between training sessions, year-round training in sports requiring lower back extension, and insufficient core strengthening.

Symptoms of spondylolysis include lower back pain exacerbated by activities involving back extension, central back pain, and pain that may radiate down the leg if symptoms persist over time.

Diagnosis of spondylolysis is conducted by a sports medicine physician, who will perform a thorough history review and physical examination. X-rays may be ordered, and if necessary, advanced imaging such as MRI or CT scans may be utilized to confirm the presence of a stress fracture.

Treatment typically involves a period of rest and rehabilitation, culminating in a gradual return to activities. A back brace may also be recommended. To prevent spondylolysis, strengthening of the core and buttock muscles is advised, along with monitoring training schedules to ensure adequate recovery time.

For athletes, a return to play is generally recommended after a rest period of 4-6 weeks and rehabilitation for another 4-6 weeks, provided the athlete is pain-free.

The American Medical Society for Sports Medicine (AMSSM), a multidisciplinary organization of sports medicine physicians, emphasizes the importance of both prevention and proper management to facilitate athletes’ recovery and maintain their health.

For more detailed information or to find a sports medicine physician, you can visit AMSSM's website at www.amssm.org.
