I'm Brett DeGoyer. I'm a primary care sports medicine specialist and neuromusculoskeletal medicine specialist practicing for Samaritan HealthCare in Moses Lake, Washington. In this case, we are discussing a high ankle sprain that occurred on the field of play. Initial management or acute management for osteopathic manipulative treatment should include very gentle mobilization of the tissues, working with effluage, which is a soft type of tissue and milking motion to help with lymphatic drainage to prevent a whole lot of swelling and promote better vascular drainage as well. After the acute phase, working with the rehabilitative phase or the recovery phase, then I would focus more on joint articulatory treatments to get motion back, as well as muscle energy to help promote getting rid of that stiffness from being locked in immobilization for whatever period of time was necessary.
