Pdf Summary
The consensus statement on electrocardiographic (ECG) interpretation in athletes, as published in the British Journal of Sports Medicine, aims to address and offer guidance on the appropriate evaluation of ECG abnormalities in athletes, which can indicate underlying conditions associated with sudden cardiac death (SCD). Sudden cardiac death is noted as the leading cause of mortality in athletes during sports, often caused by hereditary, structural, or electrical cardiac disorders.

A group of international sports cardiology experts convened in 2015 in Seattle to update ECG interpretation standards for athletes, drawing on the latest research to refine criteria that help differentiate normal physiological adaptations from pathological findings. Their recommendations were based on new data, aimed at enhancing the sensitivity and specificity of ECG use while minimizing false positives.

The criteria they defined categorize ECG findings into normal, borderline, and abnormal groups. Normal findings, which do not necessitate further evaluation, include increased QRS voltage due to ventricular hypertrophy and marked by regular athletic physiological changes. Borderline changes include axis deviation and criteria for atrial enlargement, which usually do not suggest pathology unless multiple borderline changes are present.

Abnormal ECG findings warranting further investigation include T-wave inversions, ST segment depressions, and pathological Q waves, which may suggest serious underlying heart conditions such as hypertrophic cardiomyopathy or arrhythmogenic right ventricular cardiomyopathy. In addition, conditions like Wolff-Parkinson-White Syndrome and prolonged QT intervals, also flagged on ECGs, could indicate electrical system abnormalities necessitating more diagnostic attention.

These guidelines are endorsed by numerous international sports and cardiology organizations and have streamlined methodologies for ECG interpretation to assist physicians in recognizing athletic heart adaptations from possible pathological signs, improving care for athletes worldwide.
