Patellar tendinopathy, also called “jumper’s knee,” is an overuse injury of the patellar tendon, the thick band connecting the kneecap to the shinbone. It happens when repeated stress causes tiny tendon tears and gradual degeneration rather than true inflammation, which is why “tendinopathy” or “tendinosis” is preferred over “tendonitis.”<br /><br />Common risk factors include repetitive running and jumping, frequent cutting or pivoting in sports, and sudden increases in training volume or intensity. Typical symptoms are pain in the front of the knee, especially with jumping, changing direction, or slowing down. A doctor may find tenderness below the kneecap and along the tendon, and sometimes swelling. X-rays may be used to check kneecap alignment and rule out other problems such as calcium buildup, arthritis, or fracture.<br /><br />Treatment usually begins with rest and reducing stress on the tendon. Stretching and strengthening the buttocks, hips, quadriceps, and hamstrings are important. Ice, knee straps or braces, and sometimes anti-inflammatory medicine may help with pain and swelling. As symptoms improve, rehabilitation focuses on eccentric exercises, which strengthen the muscle while it lengthens and may help heal the tendon. Recovery is slow and often takes several months.<br /><br />To prevent injury, athletes should use proper leg and core strengthening and gradually increase training. Return to sport is usually allowed when the athlete can run, squat, and hop on the injured leg without pain, while continuing strengthening exercises.
