Video Summary
A 17-year-old male developed left great toe pain after a soccer injury. Exam suggested either fracture or first MTP capsuloligamentous injury (“turf toe”). The video reviews first MTP sesamoid anatomy, including the plantar plate, sesamoid phalangeal ligaments, collateral ligaments, and tendon attachments. Initial foot radiographs showed no fracture or dislocation. MRI then demonstrated a high-grade partial tear of the medial sesamoid phalangeal ligament and a lower-grade partial tear of the lateral sesamoid phalangeal ligament, with intact intersesamoid ligament. Radiographs are first-line; MRI is the gold standard when suspicion persists.
