This study reviewed sudden cardiac death (SCD) among NCAA athletes over 10 years (2003–2013) using multiple sources, including NCAA records, media-based reporting, and insurance claims, with expert review of autopsy and case details. Among 4,242,519 athlete-years and 514 total student-athlete deaths, accidents were the leading cause of death, but SCD was the most common medical cause, with 79 deaths overall (about 1 in 53,703 athlete-years).<br /><br />Risk was not evenly distributed. Male athletes had about triple the risk of female athletes, and black athletes had about triple the risk of white athletes. Basketball players, especially Division I men’s basketball players, were at the highest risk; their SCD rate was about 1 in 5,200 athlete-years, and their 4-year career risk was about 1 in 1,300. Football and men’s soccer also showed elevated risk.<br /><br />The study found that the most common autopsy finding was autopsy-negative sudden unexplained death (AN-SUD), not hypertrophic cardiomyopathy (HCM), which was confirmed in only a small number of cases. Other causes included coronary artery anomalies, myocarditis, and coronary artery disease. This challenges the common belief that HCM is the dominant cause of athlete SCD.<br /><br />The study also showed that how cases are identified matters greatly. Media reports captured many high-profile Division I deaths but missed more lower-profile cases, while insurance claims identified only a small fraction of SCDs. The authors conclude that standardized autopsies, expert cardiac pathology review, and possibly postmortem genetic testing are needed to improve accuracy. They suggest more intensive cardiovascular screening may be justified for higher-risk groups, especially male basketball players.
