A 43-year-old female runner had 10 days of right ankle pain; ultrasound showed a small tibiotalar effusion and periosteal hypervascularity, while initial radiographs were negative. MRI revealed a distal tibial stress fracture with linear T1 hypointensity, surrounding T2 marrow edema, small effusion, and pre-Achilles fat pad edema; an incidental physeal scar was noted. A second case (19-year-old male) had normal radiographs but MRI-confirmed medial tibial stress fracture, graded using the Fredrickson system (grade 4a).
