Ryan Nussbaum, a sports medicine physician at UPMC, reviews an ultrasound consult case of peroneal tendinopathy. Short-axis ultrasound images identify the peroneus longus, peroneus brevis, and fibula. As the scan traces from the distal lower leg to the lateral malleolus, both tendons enlarge, consistent with tendinopathy. Heterogeneous and hypoechoic/anechoic fluid signals around the tendons indicate tenosynovitis and swelling beneath the peroneal retinaculum. There is concern for retinacular disruption with disorganization near the lateral malleolus. Color Doppler shows surrounding hyperemia, supporting an inflammatory process.
