I'm Brett DeGoyer. I'm a primary care sports medicine specialist and neuromusculoskeletal medicine specialist practicing for Samaritan Healthcare in Moses Lake, Washington. So in this case, we're dealing with adhesive capsulitis or frozen shoulder. For manual treatment or osteopathic manipulative treatment, my focus would be on really getting at the capsule, so the glenohumeral joint. Initially, I will do a deep tissue treatment targeted at the capsule, starting at the posterior glenohumeral joint, going to the subacromial space regions, and then anteriorly. Then I would move on and do a muscle energy treatment technique, specifically titled the Stages of Spencer, which run through extension, flexion, then circumduction of the glenohumeral joint with compression and traction, then going to treatment with external and internal rotation, and then concluding with the final stage of milking motion through the glenohumeral joint. The last thing that I would do in my office for osteopathic manipulation for a frozen shoulder would be to use myofascial treatment along the scapula, treating multiple directions in protraction, retraction, as well as rotation.
